This essay will reflect upon an incident in practice when I administered a drug to a child. I will use Gibbs reflective model (Gibbs 1988)(see appendix 1). This model of reflection will be applied to the essay to facilitate critical thought and relating theory to practice where the model allows. The drug that I have chosen to reflect upon is Fragmin (see Drug Profile 1 Appendix 2) which was administered as a parenteral subcutaneous injection to a 14 year old girl, who shall remain anonymous for the purpose of patient confidentiality in accordance with the regulations of the NMC (2008). I have chosen to reflect on this drug as it was the first injection I had given during my training which encouraged me to further develop my knowledge within this area. The first stage of Gibbs model (1988) of reflection requires a description of events (see Appendix 3). The next stage of Gibbs (1988) reflective cycle is related to thoughts and feelings aroused during the event which I can use to reflect upon (see appendix 4). The third stage of Gibbs (1988) model of reflection encourages exploration of both positive and negative experiences encountered and I have chosen a few that I propose to discuss further within this reflection. Throughout the incident the correct hospital policy was followed by my mentor in relation to preparing and administering an injection. However I was not familiar with this policy which could have negatively affected my practice as Grey (2008) suggests, that when administering medication by the parenteral routes, familiarization with local drug administration policy is essential. The patient’s initial refusal of the injection into the abdomen made me think about the child’s rights in refusing to consent to the administration of a medication as I was aware that the law states that anyone under the age of 18 is a child (Children Act 1989, section 105) and as such there are certain matters with which they are not able to make their own decisions. The patient’s refusal to allow me to use the abdomen as an injection site also made me question my knowledge base on this subject, as I did not know where I would have injected in the abdomen if consent had been given. I also realized at that point that I have no knowledge on why different injection sites are used and for what purpose as studies suggest that the selection of the injection site will vary depending on the size and age of the child (Cocoman, 2008). It was only through my mentor’s guidance that I felt confident on allowing the change of the site. Stage four Gibbs (1988), is the critical analysis. The patient was lying in bed with no relatives present and I asked consent before administration. The patient became a little distressed at this point and refused to have the injection in her abdomen saying it was too painful, however she did consent to me using her upper arm for the site and after onfirming this with my mentor I proceeded to administer the medication with no further complications. Appendix 4 My initial feeling after being asked to administer the Fragmin injection was apprehension, as I had not administered an injection before in practice. As I was administering it under the supervision of my mentor this made me feel very nervous and self conscious, however I also acknowledged the fact that I should appear confident and at ease in front of the patient as she was herself showing signs of distress. After the patient’s initial refusal I began to doubt my competence of skills and considered asking my mentor to take over, however my mentor put me at ease by explaining that if the patient consented, then it would be fine to proceed with the injection in the upper arm. After the administration, the patient thanked me for not hurting her which immediately boosted my confidence and left me feeling extremely content with the knowledge that I had performed my first injection correctly. The medical necessity of continued hospitalizing is primarily determined by the presence of an acute health condition of sufficient severity that ongoing diagnostic or therapeutic intervention, or careful monitoring, is required. However, patients often appropriately remain in the hospital when these criteria are not met, due to the lack of a suitable alternative setting to provide necessary care or other social factors. Mature discharge or discharge to an environment that is not capable of meeting the patient’s medical needs may result in hospital readmission. The Dalits of India Essay I had been planning my trip to India for a year. I read about the Dalit people by accident one day while waiting for my car at a car repair shop. The Dalit people — also known as â€Å"Untouchables† — have been the most oppressed caste for over 3,000 years, living at the bottom of Indias rigid social order. The word â€Å"Dalit† means â€Å"broken, ground-down, downtrodden, or oppressed. † Dalits comprise about  ¼ of the population of India: that’s seven times the entire population of Canada. [1] This large group of people are considered to pollutants to the rest of India. They are considered to be a lower class of people and are treated this way. The Dalit people are only given disgusting jobs like preparing bodies for cremation, picking up human waste and dead animals. The Adivasis is the collective name used for the many tribal peoples of India. Adivasis are not an homogenous group — with over 200 tribes speaking over 100 languages, which vary greatly in ethnicity, culture and language; however there are similarities in their way of life and generally perceived inferior position within Indian society. There are over 50 million Adivasis constituting 7. 5% of the Indian population, thus making it the largest tribal population in the world. 2] The best way for me to prepare for this trip was to get back to the basics. I started by weaning myself off of modern day conveniences. I packed away my coffee pot and my gps. I learned how to use a compass and a map. I took a few camping trips to learn how to start a fire and cook food on that fire. I researched the native plant life of India so I will know what I can eat and touch. I packed my luggage 6 months early. Every week or two I would go through and re-pack, getting down to the basics. It is an agriculture based agrarian farm culture. [5] These groups of people are so hated that officers that are paid to protect and enforce laws never bother. They are just there for the pay. The ‘outside’ world does not want to learn about these people. If they were just given a chance, other would learn that they are not lower class people because they are poor or live a very simple life. They do not show any relevance to the modern communist economy. They have collective ownership of the means of production, they have a sense of equality among community members, they have a spirit of cooperation with each other, they make consensus decisions pertaining to the community and aim to protect and nurture their environment. With all the planning and weaning I did to prepare, I was still amazed when I first arrived. Beautiful and sad. The people are treated like a lower class but they are so full of love and life. I learned so much about life in just the few months that I was there. Even when life sucks, you can make the best out of it. Even though these people are suppressed and treated very poorly, they survive. With the help of foundations like the Dalit Freedom Network, they will continue to survive and their lives will slowly get better. I will continue to pass the story of the Dalits along to everyone I come in contact with. Their story is an encouraging one. It is my hopes that my story will touch the heart of at least one person, so that someday, other peoples like the Dalits will people who care.

The Boy King’s Treasure

One of the most famous of all of the Egyptian kings or pharaohs in history is the boy king, Tutankhamen. Tutankhamen was not known for his accomplishments (even though he opened up religious tolerance which was absent during his father’s reign) or great building projects, but rather the artwork that came out of his burial chamber in the Valley of the Kings. His tomb is unique because most of the treasures and art was untouched and still intact. The treasure that was put in the tomb showed the wealth that would be buried with the pharaohs and kings, and how much wealth the ancient Egyptian culture has been lost to modern times due to tomb raiders. There are many symbols that show the religion of the ancient Egyptians within the tomb such as the hawk symbol for Horus. The detail that was also shown on the death mask, sculptures, and even the coffin is a true marvel of metal works. Tutankhamen was a king during the New Kingdom period of the 18th Dynasty. The death of both kings some two years later resulted in the accession of Tutankhaton, as Tutankhamun was then called, when he was about nine years old and married to Ankhesenpaaton, Meritaton’s younger sister. †[1] Tutankhaton was also known to have been disfigured and weak, but by the painting and statues of him you would never guess. Tutankhaton did not rule long over Egypt dying at the aged of nineteen. Tutankhaton unlike the kings of the past with the giant p yramids, he was buried with many other kings of his time in the Valley of the Kings. The Valley of the Kings is, â€Å"which is a rocky desert west of the Nile opposite Luxor,†[2] this took fewer resources and had more room for the kings to have their tombs put into. Figure 1. The idea of this is to show the religious aspect where the King hopes to be able to rise from the dead just like Osiris. The picture depicts also a ritual called the opening of the mouth. This was link to both birth and death on someone. Historians were able to connect that the opening of the mouth ritual through Tutankhamen’s tomb and the pyramid texts that the opening of the mouth ritual has been around since the Old Kingdom in Egypt. â€Å"Although flint pss-kfs are rare after the beginning of the First Dynasty, models of the implement, often made of other types of stone, are known from Old Kingdom tombs and continue to occur occasionally as late as the Eighteenth Dynasty. [3] This was a great discovery for the religious and belief system of ancient Egyptians. This picture also show that the Canon law which was used when Egyptians depicted people, which was used in the Old Kingdom was still in use during the New Kingdom during Tutankhamun. The funerary mask shows the amount of wealth the kings and pharaohs gathered over time. The materials that were used within the mask are made of solid gold and other semi-precious stones. The demotions of the funerary mask are, â€Å"height 54. 0 cm, width 39. 0 cm, depth 49. 0 cm. †[4] The shape of the mask is believed to be a pretty good depiction of what Tutankhamun looked like when he was alive. The rather narrow eyes, fleshy lips, and shape of the nose and chin all agree with the features visible on his mummy. †[5] This mask would have been buried on King Tutankhamun in the coffins for there were three coffins that were stacked inside one another (kind of like the Russian nesting dolls). All the coffins also had the almost identical mask piece as the funerary mask. The coffins were also made with extreme care and precision, whi ch includes both wood work and metal work. Figure 3. This piece of art also shows the â€Å"Egyptian sculptor’s ability to represent realistically the poise of the human body in the course of movement. †[6] The rest of the statuette was left out was a hippopotamus of Seth which was what Tutankhamen was harpooning. It was left out for a simple reason that it may be dangerous to the king in burial place of the king in the afterlife. â€Å"This gilded wooden figure represents Tutankhamun as Horus throwing a harpoon at one of Seth’s hippopotami. †[7] The perception that the king was the embodiment of Horus was still around from the beginning of the Kingdoms. Thanks to the discovery of the boy king’s tomb historian are able to see religious aspect of the ancient Egyptians and the wealth that has been lost through history. With the art work you can also see the masterful work of metal work that Egyptians mastered over a long time of metal working. Historians can also see that the times during Tutankhamun’s rule that the Egyptian were still using the canon of proportions as the Old and Middle Kingdoms periods. Andragogy emphasises the value of the process of learning, It uses approaches to learning that are problem-based and collaborative, and also emphasises more equality between the teacher and learner. Knowles identified the six principles of adult learning outlined below: 1. Adults are internally motivated and self-directed – Your role is to facilitate a students’ movement toward more self-directed and responsible learning as well as to foster the student’s internal motivation to learn. 2. Adults bring life experiences and knowledge to learning experiences – Adults like to be given opportunity to use their existing foundation of knowledge and experience gained from life experience, and apply it to their new learning experiences. 3. Disadvantage: the difficulty you experience when only texts and speeches are available for learning, without any visual aids. The Auditory Style – Some people prefer to learn by hearing what they want to learn. Theirs is the auditory learning style. To learn, such people would prefer listening to discussions, talking matters over, reading out of texts or making use of e-courses containing audio recordings. Advantage of this style is that you assimilate and retain information without having to see it in texts or pictures. Disadvantage: the difficulty of learning among silently reading learners EG in a Library. The Read/Write Style – If you learn best by reading texts or writing down notes from what you read, see or hear, then you are a read/write learner. Read/write learners need writing materials to take down points they think important from what they read, hear or see. Advantage of making them more self-dependent because with their note taking, they can learn much by themselves. Disadvantage of not being able to learn easily where the only  medium of instruction is visual or audio, or where they do not have access to writing materials. The Kinesthetic Style – Kinesthetic learners prefer to learn by moving and doing. They prefer interactive learning, learning through practical challenges and hands-on experience and taking in information as they move from one place to another. Kinesthetic learners are therefore not comfortable sitting in a place for long. Advantage of exposing learners faster to practice and evidence: You learn as you practice and practice what you learn; you see the evidence of what you had digested with difficulty from texts or discussions. Disadvantages where there are no places to move to for such live experience and nobody to interactive with. Learning Resources Instructor-led training remains one of the most popular training techniques for trainers. There are many resources used including: whiteboard, power point presentation etc†¦ Advantages  Instructor-led classroom training is an efficient method for presenting a large body of material to large or small groups of employees. It is a personal, face-to-face type of training as opposed to computer-based training and other methods we will discuss later. It ensures that everyone gets the same information at the same time. It is cost-effective, especially when not outsourced to guest speakers. Storytelling grabs people’s attention. Disadvantages Sometimes it is not interactive. Too much of the success of the training depends on the effectiveness of the lecturer.
